Dear community members:

The Green River Valley Health Foundation would like to thank our Sublette County community for your continued support over the years.

After ongoing negotiations with the Sublette County Rural Health Care District and Hospital District, we have decided to remain an independent foundation, focused on supporting a wide range of health and wellness services for all of Sublette County. This decision did not come easy, as we put in a lot of time and thought as to what was best for the community as a whole, including the hospital district, the foundation and the citizens of Sublette County.

As a foundation, we want to make sure that we are able to meet the needs of our community in advancing health and wellness, as well as being able to continue to support our clinics and the future critical access hospital. After much consideration, we felt keeping our independence would best help us continue to accomplish this.

There will be no changes to the structure of the Green River Valley Health Foundation or the programs that we offer, including our Families in Need program. You can now reach out directly to the foundation or visit our website to apply for funds online.

Please feel free to reach out with any questions or concerns you may have.
